ABN - Upgrade Option (Non-assigned) -- Information Form

Note: VGM's Billing and Reimbursement Advisor - US Rehab, Peggy Walker, has developed these additional forms to assist VGM members. The forms should ensure that your company gives beneficiaries complete information regarding the upgrade options, such that all parties make an informed decision about the ABN process. They can be used "as is" or you may reprint, reformat or reedit them as needed.

We believe it important that you clearly indicate the out of pocket expense that your beneficiary will incur for both the non-upgraded and upgraded equipment options.

There is discussion that the CMS will likely closely review this new ABN process. Should an inspection occur at your location, these completed forms may assist you via proof the beneficiary was aware of the expenses and other information.
